Charting the LifeCourse

Charting the LifeCourse (CtLC) helps individuals and families of all abilities and ages develop a vision for a good life. What do you need to know and do? How can you find or develop supports? What does it take to live the life you want? CtLC helps individuals and families think through these questions, and the framework is the foundation for supporting a community of learning that champions transformational change through knowledge exchange, capacity building, and collaborative engagement.

Principles

The framework is designed to help any citizen think about their life, regardless of age and ability. At the core of the framework is the belief that everyone has the right to live, love, work, play, and pursue their life aspirations.

  • Focusing on all people

  • Recognizing the person within the context of their family

  • Trajectory of life experiences

  • Achieving life outcomes

  • Holistic focus across life domains

  • Supporting three buckets of need

  • Integrated services and supporters

  • Transformational policy and systems change

Ambassadors

CtLC Ambassadors are trained and certified in the CtLC framework and are committed to ongoing learning, sharing, and contributing to person and family-centered practices.

Community of Practice

The Community of Practice for Supporting Families of Individuals with Intellectual & Developmental Disabilities develops systems of support for families throughout the lifespan of their family member with intellectual and developmental disabilities. This CoP is unique in Ohio because it focuses on all families with a member with a disability, not just those who receive formal supports.

Trainings

We provide training in Charting the LifeCourse for yourself, your family, a group of parents, a school, an agency/provider, your business, or county board.

“It’s not quite so difficult as learning a new language, but it was a new way of thinking for us. It was most definitely worth the time and effort, though, because now virtually every new experience, relationship, idea or plan Shannon encounters fits somewhere within the framework and is more easily assimilated into her life vision.”

“I just wanted to say thank you to the LC trainers at Synergy for allowing me to be open enough to explore the idea of my daughter driving. We are now enrolling her into drivers education!”

“I wish we had this info in the 60s and 70s. Re-emphasized my brother is more than his disability. I will work with J to plan for retirement and aging.”

“This helps organize my daughter’s needs, wants, good life to advocate for her in team meetings.”
